The number one goal of our home health team is to help you and/or your caregiver learn to manage your healthcare needs. Your home health caregiver will work to ensure you are actively involved in the care planning process.

Our home health services

Our clinical team provides a wide variety of services in the home setting. Care is customized to meet the patient's needs based on the doctor's orders.

Nursing services:

  • Patient education
  • Skilled observation and assessment
  • Direct care such as wound care, catheter care, etc.
  • Diabetic care 
  • Medication teaching and/or administration
  • Ostomy care

Rehabilitation services:

  • Physical therapy
  • Occupational therapy
  • Speech and language therapy

Specialty services:

  • Certified wound and ostomy care nurses
  • Total hip/total knee program
  • Registered dietitian
  • Certified diabetes educator
  • Home IV therapy
  • Heart Failure program

Other services:
Medical Social Services is involved to connect patients and their families with community resources and funding. Home Health Aides are available to assist with personal care needs such as bathing and dressing, if the patient qualifies.

How to qualify for home health

Several qualifications must be met in order for home care services to be covered by Medicare, Medicaid and most private insurance companies including:

  • Patient must be under the care of a doctor who has given orders for home health services.
  • Patient must have had a visit with their doctor for the reason home health is needed. This visit must occur up to 90 days prior to home health starting or within 30 days of the first home health visit.
  • Patient must be homebound, which means they are unable to leave the home without assistance to receive medical care.
  • Patient must have a need for intermittent skilled services of a nurse, physical or speech therapist.
  • Live in Berkeley, Charleston or Dorchester counties.

Additional admission criteria may apply depending on insurance coverage guidelines or type of service to be provided.  Our staff will be glad to assist you with verifying your eligibility for home health services and insurance coverage.
